SwimAmerica School Year 2011-2012 Private Lessons
SwimAmerica Private Lesson Information:
•Open to any swimmer 4 years old and over.
•1 on 1 instruction
•30-minute lessons
•Limited Class Size (4-12 participants per class)
•There are no refunds for missed lessons/sessions unless due to a medical emergency.

PL2 October 17-November 7
PL3 January 30-February 20
PL4 March 19-April 9
Class Times:
Class #1 6:40 p.m. - 7:10 p.m. Evening
Class #2 7:15 p.m. - 7:45 p.m. Evening
Class #3 7:50 p.m. - 8:20 p.m. Evening
Lesson Location:
•All Lessons will be conducted in the Forker Building Pool
Session Cost (four lessons):
•$119.00
Program Initiation Fee:
A new participant Program Initiation Fee of $25.00 is required for any enrollee that is participating in SwimAmerica for the first time. This fee is a one-time charge for each new participant and includes a complimentary T-Shirt and program Award Certificate.
New Participant Placement Guidelines:
•When registering a new enrollee, you will be asked to enter a placement level to assist us in your child’s initial lesson placement. If you are unsure of the ability level, it is best to choose a lower level.
•Please select the highest level of ability from the following:
•Level A. Non Swimmer
•Level B. Ability to place head underwater
•Level C. Front float, back float, and kicking
•Level D. Front crawl with breathing
•Level E. Advanced competitive skill beyond D
Refund Policy
Due to our limited enrollment, only medical emergency situations will be considered for a refund less a $10 processing fee. Any withdrawal from an enrolled session will be treated as a cancellation and is not refundable. Refunds will not be given for absence due to illness or other conflicts.
Required Release Form:
•Iowa State University and our insurance carrier require that all Cyclone Swim School participants have a release and medical authorization form signed prior to any enrollee’s participation. During the online registration process, you will be required to provide your electonic signature on this form.
