SwimAmerica School Year 2008-09 Private Lessons

SwimAmerica Lesson Information:
- Open to any swimmer 3 years old through
adult
- 1 on 1 instruction
- 30-minute lessons
- Limited Class Size (4-6
participants per class)
- There are no refunds for missed lessons/sessions
unless due to a medical emergency
- Sessions & Dates:
- All Sessions are 2 weeks Monday and Wednesday
- Session P1 October 20-29
- Session P2 November 3-12
- Session P3 January 19-28

P4 March 2-11
- Class Times:
- Class #1 6:45-7:15 p.m.
- Class #2 7:20-7:50 p.m.
- Lesson Location:
- All Lessons will be conducted in the State Gym Pool
- Required Release Form: PLEASE NOTE! - PARENT AND *PHYSICIAN'S SIGNATURE REQUIRED!
- Iowa State University requires that all Cyclone Swim School participants have a signed release form in our possession prior to any enrollee’s participation. You must bring this form to the check-in on the first day of the first session you participate in. This release will cover all classes from May 1, 2008 through April 30, 2009. *Please note that a copy of a participant’s school physical form may serve as the physician’s authorization if valid within one year of the starting date of SwimAmerica. NO SWIMMER WILL BE ALLOWED TO PARTICIPATE WITHOUT THIS FORM. A parent or guardian must sign the release and the treatment authorization form if the participant is under 18 years of age. Students who are 18 years old or will become 18 years old before the end of a session must also sign. In order for students to participate in any lesson, we must have this form prior to participation. - Session Cost:
A new participant Program Initiation Fee of $25.00 is required for any
enrollee that is 11 years of age or under participating in SwimAmerica for the
first time. This fee is a one-time charge for each new participant and includes
a complimentary T-Shirt and program Award Certificate.
